Introduction

The Community Program is a life-long opportunity that provides a wide variety of program options and services to its participants to both thrive in and remain living independently in the community. We value communication, relationships, and education with participants and their families; and we serve an integral role in long-term independence.

Community Program participants are individuals who have successfully completed the College Program and are living independently in Richfield, MN and surrounding areas.

Programming and services are divided into several customizable and person-centered service areas to ensure holistic coverage of well-being and safety of the participants in the program.

Individualized Services

Advisory Supports

Participants may opt-in for a variety of Advisory Supports to provide guidance and support in:
  • conflict resolution,
  • emotional support,
  • general check-ins,
  • medical coordination,
  • service coordination with both internal and external providers,
  • family communication,
  • social engagement coordination, and
  • a wide array of problem-solving supports needed to tackle unique and challenging day-to-day issues.

Independent Living Skill Supports

Community Program staff are able to support participants with prompting, guidance, and limited physical assistance in some tasks of independent living including:
  • cleaning,
  • monthly bill checking,
  • laundry reminders,
  • schedule building,
  • assistance in making appointments,
  • transportation, and
  • other basic home care and follow-through with tasks to maintain or improve participants level of independence and safety.

Wellness Supports

The Wellness team implements wellness programming and also offers customized services, including:
  • nutrition planning,
  • cooking skills development,
  • fitness coaching, and
  • motivational cues.
Supports can be delivered in many ways ranging from 1:1 support to small group or virtual supports, and are customized to meet the participant's needs.

Social Engagement Program

The Social Engagement team is committed to creating, maintaining, and enhancing programming that enable and encourages participants to be socially connected and engaged with MICC's community and in the community-at-large. We believe healthy participation in the community is the key to preventing social isolation.

Participants have access to robust social activities which offers:
  • Weekly social activities including arts and culture events, participant-led clubs, and many Minnesota seasonal favorites
  • Electives courses
  • Community excursions
  • Access to the Community Center with W-Fi and HD Cable
